Introduction

Diabetes is a complex, chronic illness requiring continuous medical care with multifactorial risk-reduction strategies beyond glycemic control.

Occur when the blood glucose level is too high as a result of deficiency of available, effective insulin or insulin action

It is not really a single disorder:

abnormalities of glucose homeostasis is associated with significant acute and chronic complications:

 heart, eyes, kidneys, nerves and resulting in various complications

  • Diabetes continues to increase in numbers and significance, as economic development and urbanization (reduced physical activity, and increased obesity)
  • Ongoing patient self-management education and support are critical to preventing:
  •  acute complications and reducing the risk of long-term complications
  • Significant evidence exists that supports a range of interventions (medical, nutrition and …) to improve diabetes outcomes
